The parameters of this transformation function are:

• JDBC Driver
  The JDBC driver class.
• JDBC URL
  The JDBC URL of the RDBMS.
• User Name
  The username for the RDBMS.
• Password
  The password for the RDBMS user.
• Table Name
  The name of the table to be looked up.
• Reference Field(s)
  The field(s) used as lookup references.
• Value Field(s)
  The field(s) whose values will be retrieved.
